I – Whether we are delivering to or picking up from the address below.

J – Any additional information we may need in order to deliver your package(s).
K – The amount of packages we are delivering.
L – The total weight amount of the delivery.
M – The person/company we are delivering to or picking up from.
N – The address of where we are delivering to or picking up from.
O – The description of the package(s) we are delivering.
P – Any miscellaneous information (ex. Price, Reference numbers, etc.)
Q – Signature of who is receiving the package.
R – The receiver’s name printed.
